Fertility treatment is a safe and effective way of achieving a pregnancy. Recent improvements in medication, micro-surgery and advanced fertility treatments have made healthy pregnancy a possibility for the patients who seek help for infertility.

►  Understanding Infertility -

1) What is infertility?

A couple is considered to be infertile when they have not achieved a pregnancy after a year of unprotected intercourse. A couple over thirty should probably seek help after six months of trying, since a woman’s ability to conceive begins to decline during her thirties.

2) What causes infertility?

It has been found that female factors are responsible in 30% of cases, male factors account for a further 30%, combined male and female factors account for 20% and the remaining 20% of cases are unexplained.

3) What are the causes of female infertility?

Common causes of female infertility -

  • An ovulation disorder (failure to release an egg every month)
  • Blocked fallopian tubes, which can occur when a woman has had pelvic inflammatory disease or endometriosis (a sometimes painful condition causing adhesions and cysts)

4) What are the causes of male infertility?

Common causes of male infertility

  • Producing too few sperms to fertilize an egg
  • Sperm that are not shaped properly or do not move the way they should
  • Has a blockage in his reproductive tract that keeps sperm from getting out
  • Sexual dysfunction
  • Others

5) What is unexplained infertility?

Unexplained infertility affects 20-25% of infertile couples. In the majority of these cases, the failure to reach a diagnosis is not due to inadequate investigations, but is probably due to other factors which cannot be assessed using conventional tests. For example, it is not currently possible to determine if the eggs are actually released at the time of supposed ovulation; if the fallopian tubes are able to pick up the eggs; if the sperm are capable of reaching the site of fertilization; or if the eggs can be fertilized by the sperm.

6) How is Infertility diagnosed?

Couples are generally advised to seek medical help if they are unable to achieve pregnancy after a year of unprotected intercourse. The doctor will conduct a physical examination of both partners to determine their general state of health and to evaluate physical disorders that may be causing infertility. Usually both partners are interviewed about their sexual habits in order to determine whether intercourse is taking place properly for conception. If no cause can be determined at this point, more specific tests may be recommended. For women, these include an analysis of body temperature and ovulation, x-ray of the fallopian tubes and uterus, and laparoscopy. For men, initial tests focus on semen analysis.

7) What are the treatments for infertility?

Some causes are treatable and normal fertility can be restored. For example, some cases of ovarian failure can be treated with medicines. Hormone problems can also often be treated. If no cause is found, or the cause is not treatable, then "assisted conception" is often possible. 1) Ovulation Induction and / or Timed Sexual Intercourse

Often referred to as “controlled ovarian stimulation”, fertility drugs are prescribed to stimulate ovum (egg) production in the woman. Ovulation induction combined with timed sexual intercourse is offered to couples when the sperm analysis is normal. There is no significant ovarian dysfunction and we do not suspect obstruction of the Fallopian tubes. In spite of being apparently healthy, some patients may still have problems with conceiving.

2) Intrauterine Insemination

One of the first options in fertility medicine, intrauterine insemination involves injecting the man"s sperm-or donor sperm-directly into the woman"s uterus. It is done by using a fine plastic tube which is passed through the cervix into the uterus. It can be timed to coincide with ovulation (about half way through a monthly cycle) in women who are still ovulating. Fertility medicines may also be given beforehand to maximize the chance of ovulation occurring.

3) ENDOSCOPIC FERTILITY ENHANCING SURGERY

The cause of infertility is due to some gynaec condition requiring surgical intervention. Several operative procedures can be performed as a part of fertility treatment to improve your chances of achieving a successful pregnancy. Conditions like fibroids, endometriosis, pelvic adhesions (scar tissue in the pelvis), polycystic ovaries and uterine abnormalities such as a uterine septum and adhesions can contribute to infertility. Endoscopic management of these conditions gives dramatic fertility enhancing results. It has a clear superiority and has almost completely replaced the traditional open surgery worldwide. Videos recorded during the endoscopic procedures is one of the most authentic ways of operative record for deciding future treatment in infertility protocol

4) IVF (In Vitro Fertilization)

IVF (In Vitro Fertilization) is the most common form of ART (Assisted Reproductive Technology) today. If the fallopian tubes are damaged or the sperm is poor, it is the only acceptable approach. It is also usually the most effective treatment for most other types of infertility as well. The eggs are fertilized in our laboratory and the resulting embryos are placed into the uterus 2 to 5 days later. This procedure achieves remarkable pregnancies even in women with hopelessly damaged fallopian tubes and even "unexplained" infertility.

5) Intracytoplasmic Sperm Injection (ICSI)

The idea of Intracytoplasmic Sperm Injection (ICSI) is to select a single motile and normal sperm and inject it inside the egg. It is especially recommended for couples who had fertilization failures in the past or for men with significantly abnormal sperm results.

6) PESA / TESA

Even men who don’t produce sperm can father a baby…….

PESA / TESA procedure is used when there is no sperm in ejaculate (so called "Azoospermia) confirmed in two independent semen assessments.

The procedure can be done under local anesthesia. For aspiration of sperm from the epidydymis (so called PESA) as well as for aspiration of sperm directly from the testicles (so called TESA) we use special needles which make the procedure less invasive

7) Embryo Freezing

During the IVF process, multiple eggs are stimulated to grow, then recovered from the ovary & fertilised. Several embryos are usually obtained following treatment and only two to three embryos can be transferred into the uterus. It is therefore possible to freeze some of these embryos which are not immediately transferred. The frozen embryos can be thawed out and transferred to the uterus providing additional opportunities for conception.

8) Blastocyst Transfer

Embryos are routinely transferred 2 or 3 day after egg collection & fertilization, which places the embryos into the uterus at an earlier stage than would occur naturally. In contrast, blastocyst transfer places the embryos into the uterus at a later stage (Day 5 or 6) which approximates the time an embryo would normally arrive in the uterus. This may allow a better harmony between embryo development and the uterine environment. Using newly developed culture media, a higher implantation rate has been observed following transfer of blastocysts (50%), compared to Day 3 (30%) embryo transfer. Since normally only one blastocyst is transferred, the chance of multiple pregnancy is also reduced

9) Embryo Donation

A couple’s infertility problem may also be due to a combination of physical factors that affect both the man and woman. If standard IVF treatment (even using ICSI) fail or offers too small chance of success, the transfer of a donor embryo into the woman’s womb may be the only way for her to conceive.

10) Oocyte Donation

(Eggs) Oocyte donation offers the only chance of conception for a woman who produces too few or no eggs, or eggs that are unsuitable for genetic reasons. Sperm supplied by her partner are injected directly into eggs donated by another woman & embryo are formed which are later transferred into woman’s uterus.

11) Sperm Banking-preserving your fertility for future

Sperm banking has revolutionized the future of assisted reproduction. It has allowed men whose fertility may be jeopardized by situations ranging from medications to illness to protect their potential to father children.

12) Surrogacy

For a woman to carry a viable pregnancy, the uterus must be both normally developed and functioning properly. Women that have a damaged or absent uterus, can experience the joy of creating a family by using a surrogate mother. Sometimes a woman may not be physically able to carry a pregnancy to term. Her uterus may be damaged and unable to support implantation and embryonic development. She might also have health problems such as heart disease that would place her at risk if she were pregnant. Surrogacy is an option for many such women

14) IVF Laboratory

The heart of a successful IVF is IVF laboratory. The quality of the laboratory environment goes a long way in ensuring formidable success rates. The high efficiency air filters and the two separate air handling units maintains high standard air quality in the laboratory thus meeting the European and Class 10,000 requirements for air quality.

There is a duplication of all laboratory equipment so that there is an effective back up system
