Orthopaedic Outpatient Department (Bone, Joint & Spine Services) is situated on the ground floor in the South Wing of the hospital. It consists of 4 consulting rooms (L1-L4), 2 procedure rooms and a fully equipped plaster room. The fully air-conditioned and spacious waiting area eases the patients. About arthritis

Arthritis is a disease which causes inflammation of the joints and thereby restricts their movements. This often leads to pain upon weight bearing, including walking and standing, redness, swelling and stiffness. The word arthritis literally means joint inflammation (arth = joint, ritis = inflammation).

Fear of joint replacement surgery

Arthritis generally affects the older population. It is the leading cause of disability in people who are fifty five and above. And, it is the number one disability not only in the USA but in Asia as well. The obesity epidemic in India, which leads to an increased body weight directly affects the weight bearing joints and thereby causes arthritis. Further, the reluctance of the Indian population towards exercise aggravates this issue.

However, not many people are willing to undergo this surgery, firstly due to their fear of whether the surgery will be successful or not; and secondly because of its cost. Joint replacement surgery if performed by an experienced team of surgeons, using the best implants manufactured by a reputed company, in a hospital which maintains good health practises is bound to give good results.

About joint replacement surgery

Joint replacement surgery is a worldwide success story and is one of the most common and successful operations performed. In this surgery, painful, and worn out parts of the joint are replaced with artificial surfaces, shaped in a way so as to allow a pain-free joint movement. This surgery corrects deformity and thereby improves alignment and appearance.

Joint replacement done for the first time is known as a 'primary' replacement, whereas when it is redone, it is known as a 'revision'. It can be planned, i.e. done as an elective procedure, or it can be unplanned, i.e. done as an urgent or emergency procedure for trauma, fracture, dislocation, infection or cancer.

However, there are some risks involved in this procedure like infection, Deep Vein Thrombosis (DVT), scar related issues and neurovascular problems in severe deformity. But rest assured, the percentage of these risks is just 5% compared to a 95% success rate. And, with an increased awareness and preventive measures like using prophylactic antibiotics, clean air theatres, space suits, clean hand campaigns, DVT stockings, early mobilisation and judicious use of drugs to thin the blood, the risk factors come down to being almost zero.

Delayed infection can be one major cause for concern and can happen many years after the index surgery is performed. Lowered immunity, local trauma leading to haematomas and high dose or long-term steroids can be the cause for it. This requires surgical removal of the old implant and a staged revision procedure. The results of such a revision surgery are satisfactory provided the accepted protocol is followed properly.

Another reason why a revision can occur is because the implant is loose or has failed due to implant wear or fracture. Also, as the typical lifespan of the implant is about 20 years, revision has to be considered, once the patient successfully completes them.

The findings of the Canadian Joint Replacement Registry (CJRR) reveal that degenerative (i.e. wear and tear) osteoarthritis is the most common diagnosis resulting in the need for 81% of primary total hip replacements and 92% of primary total knee replacements. Other common diagnoses for primary replacements include inflammatory arthritis (rheumatoid arthritis) and post-traumatic arthritis. The findings also state the correlation between weight and joint replacement. A high proportion of knee replacement patients were overweight (33%) or obese (54%). Similarly, a high proportion of hip replacement patients were overweight (37%) or obese (35%).
